Your Health seeks a Primary Care Physical Therapist to serve patients in the Gainesville region including homes, facilities, clinics, and via Telehealth. This full-time role offers an 8-hour day schedule (Mon-Fri, 8:00–5:00) and a salary-based package with monthly bonus opportunities.

Qualifications

• Completed Physical Therapy training program and licensed in the relevant state.
• Current CPR certification and active state license.
• Experience in healthcare settings preferred.

Responsibilities

• Perform comprehensive physical assessments and document findings.
• Collect health and psychosocial histories and educate patients.
• Arrange referrals and coordinate with health team and community resources.
• Market and provide community education about services.
