Job summary

Otterbein Home Health seeks a part-time Physical Therapist to travel locally in the West Liberty, OH area, providing rehabilitative care to home health patients. Eligible candidates will possess a Bachelor's or Master’s in Physical Therapy, an active Ohio license, CPR certification, and a valid driver’s license.

The role requires collaboration with RNs, SLPs, Social Workers and Aides, developing care plans, and delivering patient-centered therapies in home settings.

Qualifications

• Education: Bachelor's degree or master's in physical therapy.
• Experience: Minimum 2 years of physical therapist experience with community/home health preferred.
• Licensure/Certification: Current Ohio Physical Therapist license; valid Ohio driver's license; CPR certification.

Responsibilities

• Provide PT services per plan of care, assess functional status and contribute to developing the care plan.
• Assess patients for strength, mobility, gait, ROM and transfer capabilities.
• Instruct patients, families and caregivers in use and care of therapy equipment.
• Report reactions or changes to care to physician as needed.
• Initiate therapy programs and train other personnel and families in their therapy program.
• Supervise PTA as necessary.
• Participate in Quality Assessment/Performance Improvement program.
• Coordinate with home care team for comprehensive care.
• Submit clinical/progress notes timely per guidelines.
