Job summary

LiveWell Homecare Agency, LLC is seeking a Home-Based Physical Therapist to provide comprehensive physical therapy services in clients’ homes across the service area. Responsibilities include assessments, treatment planning, and interdisciplinary collaboration to support mobility and functional independence.

Candidates must hold a DPT, be licensed as a Physical Therapist, have a valid driver’s license, and obtain BLS within 90 days.

Qualifications

• PT licensure in the applicable state
• BLS certification within 90 days of employment
• Own a valid driver’s license and auto liability insurance

Responsibilities

• Conduct evaluations and deliver treatments such as muscle strengthening, gait training, ADL training, and device management for patients of all ages
• Create and update personalized treatment plans with measurable goals, monitoring patient progress
• Perform accurate and timely clinical documentation in compliance with guidelines
• Educate patients and caregivers on activity routines and safety procedures
• Coordinate with healthcare providers, providing updates and modifying care plans as needed
• Manage scheduling efficiently, including travel logistics and equipment transportation
• Ensure adherence to department policies, safety protocols, and quality initiatives
• Monitor insurance coverage and patient authorizations, reporting concerns to management
• Perform additional duties as assigned to support departmental goals
