Job summary

All Care and Comcare Home Health seeks a compassionate Field Physical Therapist to provide high-quality, individualized care in patients' homes with a flexible schedule and supportive team environment.

In this role you will perform SOC/ROC/Recert/DC, collaborate with an interdisciplinary team, and educate patients and families on home exercise programs while traveling across the service area.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Two professional references.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC), resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
