Job summary

Collective Therapy is seeking a Physical Therapist to provide mobile outpatient PT in South Charlotte, NC. This full-time role focuses on 1:1 patient care, with flexible scheduling and clinician autonomy.

You will conduct evaluations, design plans of care, and deliver personalized therapy in patients' homes, promoting independence and safety. New graduates are welcome and mentorship is provided.

Qualifications

• Active North Carolina PT license.
• Strong clinical judgment and problem-solving skills.
• Excellent communication and organizational skills.
• Ability to work independently with a flexible schedule.

Responsibilities

• Perform comprehensive physical therapy evaluations and treatments.
• Develop individualized plans of care.
• Provide strength, balance, mobility, gait, and functional interventions.
• Address fall prevention and home safety.
• Adapt treatment to each patient's environment and goals.
• Monitor progress and modify treatment plans as appropriate.
• Communicate with patients, families, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Build meaningful, long-term relationships with patients.
