Job summary

Twin Cities Physicians in Minnesota is seeking a Physician Assistant (PA-C) to serve as Mobile Rounding Provider for senior living communities statewide. This full-time position emphasizes geriatrics, preventative care, chronic disease management, and care coordination with on-site visits across assisted living and memory care settings.

You will perform examinations, develop and adjust care plans, collaborate with physicians, nurses, therapists, and families, and help patients navigate complex

Qualifications

• Active Minnesota PA-C license in good standing.
• Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, or post-acute care preferred.
• Strong clinical judgment and patient-centered approach.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide on-site rounding care in senior living communities.
• Perform physical examinations, chronic disease management, preventative care, and follow-up visits.
• Develop and adjust individualized care plans based on patient needs.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Communicate effectively with families and care teams regarding treatment plans and patient progress.
• Support continuity of care and help reduce unnecessary emergency room visits and hospitalizations.
• Complete documentation, medication management, and lab coordination.
