Job summary

Health Care for the Homeless is seeking an Outreach Worker - Mobile Driver to operate a mobile clinic and perform outreach in Baltimore, MD. The role focuses on safely transporting the clinic, setting up and breaking down equipment, and supporting field operations while engaging clients to connect them with care and services.

The ideal candidate will have a high school diploma or GED (associate’s degree preferred), a valid driver’s license, and at least 2 years of experience engaging vulnerable

Qualifications

• High school diploma, GED or equivalent required.
• Associate’s degree in Human Services preferred.

Responsibilities

• Safely operate and transport the mobile clinic and perform basic vehicle checks.
• Assist with mobile clinic setup and breakdown, including equipment and supplies.
• Support client flow, registration, patient check-ins/outs, referrals, and documentation.
• Conduct outreach in shelters, encampments, meal programs to connect individuals to services.
• Distribute basic needs items like hygiene kits, food, clothing, water.
• Provide basic client support and linkage to care with referrals and follow-up.
• Document outreach encounters in HMIS, AthenaOne, and required systems.
• Enter required HMIS data elements and track engagement activities.
• Help re-engage clients who disengaged from care; report findings.
• Coordinate with clinical and outreach staff for smooth daily operations.
• Maintain safe environment for staff and clients in field settings.
• Adhere to HIPAA, OSHA, infection control, and mobile clinic protocols.
