Carmel Mountain Rehab & Care in San Diego, CA is seeking a licensed Occupational Therapist for Mobile Outpatient care. This part-time role may grow into full-time hours, with a flexible schedule and the opportunity to influence care at the facility level.

You will screen and treat cognitive and ADL disorders, travel to ALFs, coordinate with the care team, and contribute to discharge planning. Competitive pay and supportive leadership accompany a dynamic, values-driven work environment.

Qualifications

• Licensed Occupational Therapist (OT) in good standing.
• Open to part-time and potential full-time hours.
• Willingness to travel to ALFs and client sites.

Responsibilities

• Screen and evaluate patients with cognitive/ADL disorders; develop plans of care.
• Use standardized assessments and evidence-based interventions.
• Provide rehabilitative, skilled treatments per regulatory standards.
• Administer ADL retraining, therapeutic activities, exercises and modalities.
• Monitor progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed for continued care.
• Provide counseling to patients, families, and care teams.
• Communicate progress and barriers with supervisors and staff.
• Prepare comprehensive discharge summaries as required.
