Qualifications

• Wisconsin Optometry License
• Must pass Department of Justice background check (LiveScan) and drug test.

Responsibilities

• Provide vision evaluations: check VA’s, assess binocular vision, IOP, pupils, perform refraction and prescribe correction, assess ocular health
• Prescribe vision devices, test patient on applicable equipment to determine best option, and instruct patient in proper use
• Confirm that patient diagnosis is properly documented in internal systems
