Job summary

Summit Home Care and Hospice in Mansfield, OH seeks an experienced Occupational Therapist to join our home health team. This on-the-road role may include training at the local office or remote work as needed.

You will evaluate functional status, develop treatment plans, coordinate with physicians, supervise assistants, and participate in discharge planning. A valid OT degree, state license, CPR, and two years of clinical experience are required.

Qualifications

• Graduate of ACOTE-accredited occupational therapy curriculum.
• Registered with the National Registration Examination (AOTA).
• Licensed driver with a reliable automobile.
• Current CPR Certification.
• Two years of clinical OT experience.
• Strong verbal and written communication and organizational skills.

Responsibilities

• Evaluate patient's functional status and determine needs.
• Assess home environment for hazards and equipment needs.
• Develop and implement the total plan of care.
• Perform work capacity evaluation when applicable and refer to training.
• Create treatment programs and set achievable goals.
• Communicate care plans to physicians and team members.
• Instruct patients and caregivers in daily activities and device use.
• Supervise OT assistants as per policy and state regulations.
• Assist in discharge planning and care coordination.
