Job summary

Mercy Medical Center Home Health and Hospice is seeking an Occupational Therapist to provide skilled home-based therapy under physician direction. You will educate patients and families, ensure safety, and document care in compliance with Medicare guidelines. • Occupational Therapist with current licensure in the state of employment.

Responsibilities

• Plan of Care: Develop and adjust the OT plan of care as directed by the attending physician.
• Evaluation: Assess patient functioning and progress using appropriate measures.
• Patient Outcomes: Improve range of motion, strength, and coordination through therapeutic activities.
• Minimize Patient Risk: Teach preventative measures to reduce hospitalizations.
• Family Educator/Advocate: Educate patient and family on disease/injury and home care.
• Supervision: Oversee COTA care and share supervisory visit results.
• Policies: Complete documentation per agency protocol and Medicare guidelines.
• Rules and Regulations: Follow agency policies and communicate schedule changes.
• Operations: Attend in-service trainings and mandatory meetings.
