Job summary

Telecare Corporation is seeking a Case Manager for its 24/7 Mobile Crisis Response Team (MCRT) serving San Joaquin County. You will partner with a clinician to respond to behavioral health crises in the community, deliver immediate, short-term services, and help stabilize and connect individuals to ongoing care while aiming to divert hospitalizations and justice involvement.

• Master's degree in Social Sciences
• One year of mental health experience
• Valid driver's license and personal vehicle insurance with your name listed as a driver
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.

Responsibilities

• Respond to behavioral health crisis anywhere in the community with a clinician.
• Provide immediate, field-based, and short-term intensive services to individuals experiencing a mental health or substance use crisis.
• Stabilize and de-escalate individuals in the community, connect them to ongoing care, and divert them from unnecessary hospitalization or involvement with the criminal justice system.
• May be periodically assigned to take calls, and screen calls for a mobile team's response.
