Job summary

• Evidence of clinical experience providing direct patient care (practicum, internship or post-graduate).
• Ability to bend, stoop, climb, drive between locations and lift up to 5 pounds.

• Coordinate with Highway to Hope program and collaborate with rural agencies and other providers.
• Ensure timely response to service requests and maintain QA standards.
