Job summary

As a mobile clinical resource, you’ll document care, review orders, and assist with lab follow-ups, referrals, and medication procedures while collaborating with residential staff and leadership.

Qualifications

• Current New Jersey LPN license required.
• Experience with IDD preferred.
• Background in residential/community health care.

Responsibilities

• Proactive vitals monitoring, weights, blood sugars, and other measures; escalate changes as needed.
• Provide enhanced clinical oversight to higher‑medical‑need homes and track labs, referrals, imaging, follow‑ups, and recommendations.
• Prepare individuals for medical visits with accurate histories, vitals, and updated medication lists.
