Job summary

Bird's Eye Medical is seeking an Outreach and Referral Coordinator to build relationships that enable mobile clinics across Clallam, Grays Harbor, Jefferson, and Pacific Counties. This field-based role requires frequent travel and coordination with tribal health programs and community partners.

You will handle scheduling, telehealth support, and referral follow-through, ensuring smooth operations at host sites and through the healthcare network.

Qualifications

• 1–3 years in community health, outreach, telehealth, or health scheduling.

Responsibilities

• Develop and maintain relationships with host sites and tribal health programs.
• Coordinate event logistics with host organizations and accessibility checks.
• Manage outreach materials and channels (flyers, calendars, social media).
• Identify local healthcare providers and support referrals with warm handoff and follow-through.
• Schedule telehealth appointments and manage provider calendars.
• Administer the telehealth platform with setup and on-site support.
• Track outreach activity, attendance, and referrals in BEM's reporting system.
• Participate in monthly coordination calls and travel to mobile clinic sites across the four-county region.
