Job summary

Maternity Care Coalition (MCC) in Philadelphia is seeking a Full-Time Professional to provide culturally sensitive group education and case management to pregnant and new parenting families. You will perform assessments, referrals, follow-up, and home visiting services during hours including evenings and weekends.

The role emphasizes collaboration with community resources, accurate documentation, and maintaining confidentiality while supporting families to achieve health and social goals.

Qualifications

• Two-plus years of direct services in community outreach.
• Bachelor's degree in Social Work, Public Health, or related field preferred.
• High school diploma or equivalent required.
• Ability to travel in the Greater Philadelphia region for home visits and outreach.
• Strong interpersonal and documentation skills for client interactions.

Responsibilities

• Outreach to locate pregnant women and refer to health and social services.
• Provide intensive case management with assessment, referral, follow-up, and monitoring.
• Support clients in planning for health and social service needs.
• Coordinate prenatal, postpartum, well-child, and early intervention appointments.
• Conduct client education and distribute educational materials.
