Job summary

Savoy Life is seeking a dedicated Registered Nurse (RN) to provide hands-on clinical support for seniors in homes, assisted living, and memory care. The RN will identify care gaps, implement chronic care plans, and coordinate with primary care providers and families across multiple settings.

The role requires an active RN license in WA or ID, valid driver's license, and willingness to travel between Moscow/Lewiston, ID and Pullman, WA.

Qualifications

• Active RN license in WA or ID.
• Experience in geriatric care and care coordination.
• Strong documentation and EMR skills.
• Willingness to travel between Moscow/Lewiston, ID and Pullman, WA.

Responsibilities

• Conduct assessments and follow-up care for residents in senior settings.
• Assist Advanced Practice Providers in developing chronic care plans.
• Coordinate care with primary providers, specialists, and families.
• Partner with local coordinators and virtual providers to support care.
• Maintain accurate EMR documentation and reporting.
• Triage after-hour calls to support resident care.
