Job summary

Western Geriatrics and Neurology is seeking compassionate NP/PA to deliver mobile primary care to seniors in homes, SNFs, and assisted living communities. This role offers a flexible schedule, autonomy, and a base salary plus productivity within Brigade Health.

You will perform comprehensive assessments, manage chronic and acute conditions, and coordinate with caregivers and interdisciplinary teams to provide patient-centered care. Strong communication and a valid driver's license are required.

Qualifications

• Active NP or PA license.
• 1+ years of primary care, geriatrics, home health, hospice, SNF, or senior care experience preferred.
• Strong communication and clinical skills.
• Valid driver’s license & reliable transportation.

Western Geriatrics and Neurology, part of Brigade Health, is hiring compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality mobile primary care to older adults in the comfort of their homes, skilled nursing facilities, assisted living, and senior communities.
