The position emphasizes patient education, crisis response, and relapse prevention planning, with duties spanning education, advocacy, housing support, and coordination of care plans under supervision.

Qualifications

• Crisis intervention experience and the ability to assess urgency in high-stress settings.
• Experience delivering psychoeducation and coordinating care plans.
• Knowledge of relapse prevention and disease management strategies.

Responsibilities

• Provides psychoeducation as indicated in the PCP.
• Assists with crisis interventions.
• Assists the team leader with behavioral and substance use disorder interventions.
• Assists with the development of relapse prevention and disease management strategies.
• Participates in the initial development, implementation, and ongoing revision of the PCP.
• Communicates the beneficiary’s progress and the effectiveness of the strategies and interventions to the team leader as outlined in the PCP.
