• Screen telephone referrals and perform psychosocial evaluation and assessment.
• Develop and implement short-term service plans for patients.
• Provide counseling, care management and referrals for long-term treatment.
• Prepare case histories and maintain records per program requirements.
• Coordinate with community agencies to ensure continuity of care.
• Participate in interdisciplinary team meetings and share information with providers.
