VNS Health in New York, NY, is seeking a Social Worker to provide direct psychosocial services to mentally ill patients in the community, along with linkage, coordination and follow-up with service providers.
The role involves screening referrals, evaluating needs, developing short-term plans, and delivering counseling and care management to ensure continuity of care in coordination with the program team.
Provides direct psychosocial services to mentally ill patients in the community who are experiencing psychosocial difficulties. Provides linkage, coordination with, referral to and follow-up with appropriate ongoing service providers. Provides information and consultation to other community agencies and other disciplines, including other services of the Agency. Assists in the overall functioning of the Program. Works under general supervision.
The Adult Mobile Crisis program provides rapid, community-based crisis intervention and short-term case management for adults experiencing or at risk of a behavioral health crisis.

Current registration to practice as a Licensed Master Social Worker in New York State required As determined by operational/regional needs, valid drivers license may be required National Provider Identifier (NPI) number required or must be obtained upon hire. Management will provide assistance with the NPI application process required
Master's Degree In Social Work after successfully completing a prescribed course of study at a graduate school of Social Work accredited by the Council on Social Work Education and the Education Dept. and who is certified or licensed by the Education Dept to practice Social Work in New York State required
Minimum of one year experience as a Social Worker in a health care setting required As determined by operational needs, bilingual skills may be required
USD $33.85 - USD $42.33 /Hr.
