Qualifications

• Bachelor's degree with major preferred in psychology, social work, or social sciences.
• At least two years of experience preferred in behavioral health.

Responsibilities

• Link patients with aftercare services prior to discharge and aid access to outpatient services.
• Conduct crisis evaluations and coordinate referrals for treatment.
• Provide follow-up support and monitor dispositions.
• Document interventions and outcomes in the electronic health record.
• Respond to emergency calls with triage and resource guidance.
