Job summary

Easterseals PORT Health is seeking a licensed-eligible clinician for the Mobile Crisis Coordinator role in Virginia. The position provides crisis intervention, care coordination, and 24/7 crisis line support in the REACH Mobile Crisis Team, with on-site or telehealth components.

Qualified candidates will have QMHP/QDDP credentials, a relevant degree, and Virginia licensure eligibility. The role offers a full benefits package and opportunities for professional growth within ESPH.

Qualifications

• Bachelor's degree in human services or related field from USDOE/CHEA accredited institution.
• Master's degree in Counseling, Social Work, Psychology or related field preferred.
• Current, unrestricted Virginia license as LPC/LCSW/LMFT or QMHP-T eligibility.

Responsibilities

• Provide rapid response to individuals in behavioral health crisis.
• Meet individuals in comfortable environments for engagement and stabilization.
• Coordinate care, crisis intervention, and treatment planning.
• Refer to medically necessary behavioral health services and supports.
• Maintain documentation for Mobile Crisis services and CEPPs.
• Collaborate with REACH team and community partners.
