Job summary

La Casa de Salud (LCDS) in the Bronx seeks a Peer Navigator to bridge community members with clinical and supportive services. The role focuses on compassionate, trauma-informed engagement across street-based, community-based, and clinic-based outreach, with occasional backup driving for mobile outreach.

Candidates should bring lived recovery experience, strong communication, and the ability to work independently in a fast-paced setting while maintaining HIPAA compliance and confidentiality.

Qualifications

• High school diploma or equivalent required.
• Lived experience with recovery, homelessness, HIV/HCV, behavioral health, or justice involvement preferred.
• Completion of or willingness to complete Peer Recovery Training and/or certification.

Responsibilities

• Establish trusting relationships with individuals encountered through outreach activities.
• Support participants in navigating services such as primary care, behavioral health, housing, and benefits enrollment.
• Provide appointment reminders and accompany clients to appointments as needed.
