Job summary

Your Health Organization in Bennettsville, SC is seeking a dedicated Community Health Worker to serve patients in the local clinic and fields. The CHW will link patients with healthcare services, arrange visits, and support home and ALF-based care with transportation coordination.

You will deliver medications and DME, support telehealth visits, and assist with RPM setups, ensuring timely, compassionate care across settings. This is a full-time position with 12-hour shifts (7:00 AM–7:00 PM).

Qualifications

Certified Medical Assistant, Paramedic, or Licensed Nurse preferred.
Phlebotomy experience preferred or willing to complete training.
Experience in public health or home health preferred.
Experience with DME delivery or logistics preferred.
Proficient in using technology tools for remote patient monitoring, telemedicine, and EMR.

Responsibilities

Visit patients at homes and ALFs to coordinate care with the care team.
Deliver medications and DME to patients' homes, reviewing instructions.
Coordinate telehealth visits between patients, pharmacists, specialists, and providers.
Set up RPM equipment training and provide ongoing support and troubleshooting.
Coordinate with care team to ensure care plan compliance and patient needs.
Travel between offices and facilities daily with reliable transportation.
