Job summary

UPMC Center for Emergency Medicine is seeking a full-time Mobile Professional Care Manager to support the Care Management Department in Butler County. This role involves traveling throughout the community to assess and coordinate care for Health Plan members, delivering services primarily in the community and remotely as needed.

The ideal candidate will have licensure in health or human services and a master's degree (or an RN license), plus several years of behavioral health and case management

Qualifications

• PA licensure in health or human services field and master's degree or licensed RN.
• 3 years of behavioral, clinical, utilization management, home care, discharge planning, and/or case management experience.
• 5 years of community-based case management and behavioral health experience preferred.

Responsibilities

• Conduct face-to-face assessments in member's community, home, or facility.
• Coordinate with physicians to ensure follow-up and coordinated care.
• Develop and implement integrated care plans with providers and health plan staff.
• Facilitate communication among members, families, and care teams.
• Identify barriers to care and create outreach plans to engage members.
• Maintain documentation within established departmental timeframes.
