Chisago County Health and Human Services in North Branch, MN, seeks three full-time County Agency Social Worker - MnCHOICES Assessor positions in the Aging and Disabilities Division.

The MnCHOICES Assessor conducts person-centered assessments to determine eligibility for long-term services and supports and collaborates with individuals, families, and partners to develop care plans that promote independence.

Qualifications

• Bachelor's degree in social work, psychology, sociology or closely related field.
• Bachelor's degree in any field and one year of experience as a social worker in a public or private social services agency.

Responsibilities

• Interviews clients at intake.
• Carries a caseload with ongoing social casework.
• Provides counseling to families and individuals.
• Conducts complex case evaluation to determine treatment.
• Prepares intensive long or short-term treatment plans.
• Interprets programs to clients and refers to community services.
