Qualifications

• Bachelor’s degree in social work, psychology, sociology, or closely related field.
• Or a bachelor’s degree in any field with one year of social worker experience in a public or private social services setting.
• One year of satisfactory paid performance in a Home and Community Based services setting preferred.
• Public Health Nurse with at least one year of home and community-based experience or a Registered Nurse with at least two years of home and community-based experience.

Responsabilités

• Provide case management and care coordination to aging and disabled persons as assigned.
• Discretion A 75% in duties.
• Intake duties and serving in resource capacity to intake team.
• Complete MnCHOICES assessments and face-to-face reassessment screenings as required by state statute.
• Ensure documentation is timely and complete to maximize reimbursement for case management services.
