Job summary

Compass Healthcare Consulting & Placement is seeking an experienced MLTC Intake RN with UAS-NY experience for a Managed Care Plan serving New York. The role involves field and remote assessments, with home visits to members in their homes.

Openings are in the Bronx/Manhattan, Bronx/Westchester, Brooklyn/Staten Island, or Queens/Nassau territories. Requirements include a NYS RN license, BSN, and UAS-NY RN experience; bilingual ability in Chinese, Spanish, Russian, Punjabi, or Hindi is required

Qualifications

• Current New York State Registered Nurse License is required.
• Bachelor’s degree in Nursing (BSN) is required.
• One year of experience in an acute care setting is required.
• Must have UAS-NY RN experience.
• Home care and management experience is preferred.
• Proficient in MS Office.
• Strong oral, written, and interpersonal communication skills.

Responsibilities

• Conduct outreach activities within the community to assess eligibility and safety of potential members.
• Collaborate with Intake/Outreach and clinical teams to ensure appropriate services are provided to new members.
• Communicate effectively with potential members, families, referral sources, and physicians.
