Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) to conduct comprehensive in-home health assessments for Medicare Advantage and other plans. This field-based role travels across Missouri, with 8am–8pm flex scheduling and no on-call, weekends, or holidays.

The position emphasizes education, gap closure, and collaboration with PCPs to improve outcomes, while offering a strong base salary plus incentives and a robust benefits package.

Formación

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted Missouri license without disciplinary actions.
• 1+ years of clinical experience; home health or geriatric experience preferred.

Responsabilidades

• Conduct in-home assessments across Missouri, with vital signs, history, medication reconciliation and exams.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close care gaps and coordinate referrals.
• Educate patients on diseases, medications, and adherence; address social determinants of health.
