Qualifications

• Bachelor's degree in social work; or bachelor's degree in a related field with one year of social work/health and human services experience.
• Two years of related work experience in Health and Human Services preferred.
• MnCHOICES Assessor certification within six months of employment.
• QDDP status may be required upon hire.
• A valid driver's license or equivalent mobility is required upon hire.

Responsibilities

• Perform intake, outreach and assessment with communities, families and individuals and provide social service interventions.
• Meet with clients to identify needs and services; review diagnostic evaluations for eligibility.
• Develop case plans, provide services and coordinate case management activities; conduct assessments and screenings; manage crisis situations and advocate for services.
• Consult with Supervisor and County Attorney's office on legislation, data practices and ethical issues; train and advise others in the department.
• Oversee and mentor new staff, students and interns; participate in orientation activities and training.
• Report maltreatment, death and serious injuries; provide documentation for appeals and legal proceedings; maintain program resources.
