Qualifications

• Reliable means of transportation and ability to travel to multiple locations around Stearns County.

Responsibilities

• Provide case management services by assessing, planning, and authorizing provisions of services.
• Conduct comprehensive assessments and investigations to determine safety and needs, then recommend interventions.
• Maintain accurate, up-to-date client records to document case activity.
• Coordinate and plan for emergencies and out-of-home placements as needed.
• Coordinate with department and community providers to deliver services.
