Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in Minnesota.

Description

The MDS Coordinator oversees the completion, editing, locking, and transmission of all MDS assessments. This role ensures timely, accurate assessments, maximizes case mix, and maintains regulatory compliance. It involves coordinating care plan completion with the Interdisciplinary Team (IDT) and providing clinical support to nursing staff to enhance assessment accuracy and quality of care.

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in Minnesota
• Successful completion or pursuit of certification in CPR and emergency care for healthcare providers
• Strong verbal and written communication skills for directing RAI processes and resident care
• Ability to perform essential functions with or without reasonable accommodation
• Pass criminal background check as per Minnesota Department of Human Services requirements

Preferred Qualifications

• Experience in healthcare, skilled nursing, or assisted living facilities
• Experience with MDS and Resident Assessment Instrument (RAI)
