Community Care is hiring a Case Manager/Care Manager for our Family Care Program in Milwaukee, WI. This is a Monday through Friday position, 8:00 a.m. to 4:30 p.m.

Case Manager/Care Manager Job Responsibilities:

1. Manage caseload of Family Care Program members
2. Complete comprehensive clinical assessments
3. Develop, implement, and monitor member-centered care plans; provide ongoing care coordination, member education, quality assurance, and service monitoring
4. Travel locally using personal vehicle, with valid driver's license and proof of insurance

Case Manager/Care Manager Job Requirements:

• Education: Bachelor's Degree in Human Services or a closely related field required
• Licensure: Current long-term care functional screener certification preferred (eligibility required)
• Experience: Minimum of 3 years recent experience with older adults or persons with physical or intellectual disabilities; community nursing experience preferred
