Job summary

Virtua Health in Voorhees, NJ is seeking a Paramedic to perform scheduled in-home clinical care as part of the Mobile Integrated Healthcare (MIH) program. You will conduct in-home assessments, perform medication reconciliation, telehealth facilitation, and arrange safe transitions of care to reduce readmissions.

Ideal candidates hold NJ Paramedic certification with at least 1 year of field experience, plus training in telehealth, ACLS/PALS, and EVOC.

Qualifications

• NJ Paramedic certification with at least 1 year field experience.
• MIH, hospital-at-home, home health or telehealth experience preferred.
• Graduate of a recognized Paramedic Training Program.

Responsibilities

• Conduct in-home patient assessments and medication reconciliation.
• Document clinical notes accurately in EMR/ePCR.
• Facilitate telehealth visits and remote monitoring setup.
• Maintain equipment and ensure infection control after visits.
• Support safe transitions of care to reduce readmissions.
• Participate in program improvement activities.
