Job summary

Virtua Health seeks a Mobile Integrated Healthcare paramedic to perform in-home clinical care and post-discharge follow-up under the MIH program. You will conduct assessments, coordinate care, and support safe transitions from hospital to home.

Responsibilities include telehealth facilitation, documentation in the EMR, and monitoring devices, with emphasis on preventing readmissions and ensuring patient safety in the home setting.

Qualifications

• Current NJ Paramedic certification; minimum 1 year field experience.
• Graduate of a recognized Paramedic Training Program.
• Valid NJ driver’s license; annual credentials (BLS, ACLS, PALS/PEPP) and EVOC.

Responsibilities

• Conducts comprehensive in-home patient assessments and medication reconciliation.
• Identifies clinical changes and documents plan of care.
• Completes accurate clinical documentation in the EMR/ePCR.
• Facilitates telehealth visits and remote monitoring device management.
• Maintains MIH vehicles and equipment; ensures infection control.

Job Summary

Performs scheduled in-home clinical care and post-discharge follow-up as part of the Mobile Integrated Healthcare (MIH) program. Performs home safety inspection, patient assessment, medication reconciliation, point‑of‑care testing, telehealth facilitation, and equipment setup to support safe transition of care and reduce readmissions.

Position Responsibilities

• Conducts comprehensive in‑home patient assessments, medication reconciliation, and administers treatments within the paramedic scope of practice.
• Identifies clinical changes and documents plan of care.
• Completes accurate clinical documentation in the EMR/ePCR.
• Responds to quality assurance and medical oversight requests.
• Facilitates telehealth visits with providers, sets up and manages remote monitoring devices, and communicates findings to the interdisciplinary care team.
• Maintains MIH vehicles, durable medical equipment, and supply inventory; ensures infection control and equipment cleaning after each visit.
• Performs point‑of‑care testing and specimen collection (phlebotomy) per protocol and coordinates specimen transport/processing.
• Participates in patient/caregiver education, community outreach, and program improvement activities; cross‑trains with hospital‑at‑home staff.

Position Qualifications Required

• Required Experience: Current NJ Paramedic certification: minimum 1 year field experience as an active paramedic required.
• Prior MIH, hospital‑at‑home, home health, or chronic care management experience or experience with telehealth/remote monitoring platforms preferred.
• Required Education: High school diploma or GED.
• Graduate of a recognized Paramedic Training Program.
• Training / Certification / Licensure: Current New Jersey paramedic certification issued by the NJ Department of Health Current BLS CPR certification (American Heart Association or equivalent). Current American Heart Association Advanced Cardiac Life Support (ACLS) certification. Current American Heart Association Pediatric Advanced Cardiac Life Support (PALS) or American Academy of Pediatrics PEPP certification. Emergency Vehicle Operator Course (EVOC) certification. Valid driver’s license recognized by the State of New Jersey; driving record must meet insurance company standards. Must complete annual clinical competencies, Fit Test, PPD screening, AMOC training, and any other required training by designated deadlines.

Hourly Rate

$30.37 - $47.20 The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data.
