Job summary

Integra Vascular seeks a qualified NP/PA to perform on-site assessments at client facilities. You will conduct comprehensive histories and physicals for complex patients, order tests, and ensure care plans meet regulatory standards.

Responsibilities include education of families and staff, collaboration with physicians, and timely reporting. Travel to facilities and maintaining EMR/PACS workflows are essential.

Qualifications

  • Current Nurse Practitioner or Physician Assistant license.
  • Graduate of an accredited NP/PA program.
  • ACLS certification and infection control certification are required.

  • Performs on-site screening assessments to determine patient treatment plans for residents at client facilities.
  • Conducts age-appropriate history and physicals for complex patients.
  • Orders and interprets diagnostic tests as age-specific needs require.
  • Maintains regulatory compliance and monitors intervention effectiveness.
  • Educates families, clients, and prospective accounts about services and programs.
  • Completes weekly and monthly reports in a timely manner.

Responsibilities:
  • Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents at client facilities by the completion of a comprehensive assessment
  • Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
  • Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs
  • Assists with the provision of care in accordance with facility, state, and federal regulations
  • Monitors the effectiveness of interventions
  • Maintains relationships with all appropriate contacts in facilities
  • Educates family members, clients, and prospective accounts regarding services and programs
  • Assists with the development and maintenance of physician relations, as assigned
  • Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner
  • Other Duties as assigned by Medical Director
Qualifications:
  • Current Nurse Practitioner/ Physician Assistant License
  • Degree from an accredited Nurse Practitioner/ Physician Assistant Program
  • ACLS
  • Current infection control certification
  • Strong clinical collaboration with physicians
  • Proficient computer skills
  • PACS and EMR experience preferred
  • Committed to professional growth and learning
  • Clean driving record
  • Personally owned vehicle required
  • Local travel required
  • Not listed on the OIG exclusion list
Preferred Experience:
  • Vascular Care: Venous Insufficiency, Peripheral Arterial Disease
  • Image Guided Injectables
  • Advanced Geriatric
  • Skilled Nursing Facilities, Assisted Living Facilities
