  • Coordinate timely discharge plan implementation.
  • Participate in multidisciplinary rounds to review the plan of care.
  • Collaborate with healthcare professionals to remove barriers to care.
  • Communicate with reimbursement to obtain authorizations.
  • Plan strategies to reduce length of stay and improve outcomes.
  • Initiate discharge planning by assessing patient and family needs.
  • Maintain communication with all stakeholders in discharge planning.
  • Engage with primary care physicians to support continuity of care.
  • Work with community organizations to arrange housing and follow-up.

About This Role

The MetroPlus Care Coordinator is responsible for coordinating and monitoring patient-centered quality care, ensuring optimal resource utilization and compliance with regulations. This role is ideal for individuals who can work independently and are committed to improving patient outcomes.

What You'll Do
  • Review each patient's chart to ensure documentation supports the plan of care.
  • Coordinate and facilitate timely implementation of discharge plans.
  • Participate in multidisciplinary rounds to review the plan of care.

Requirements
Education
