Qualifications

Responsibilities

  • Review patient's charts to ensure documentation supports care plans.
  • Coordinate multidisciplinary rounds to discuss care plans.
  • Collaborate with healthcare professionals to remove barriers to care.
  • Communicate with reimbursement departments for authorizations.
  • Plan strategies to reduce length of stay and improve outcomes.
  • Initiate discharge planning by assessing patient needs.
  • Coordinate implementation of discharge plans and follow-up care.
  • Perform post-discharge phone calls to verify care linkage.
  • Maintain communication with stakeholders in discharge planning.
  • Engage with primary care physicians to support continuity of care.

About This Role

  • Department/Unit: PROFESSIONAL REV ORG
  • Schedule: Full-Time, Day (9:00 A.M – 5:00 P.M)
What You'll Do
  • Review each patient's chart to ensure documentation supports the plan of care.
  • Coordinate and participate in multidisciplinary rounds to discuss care plans.
  • Collaborate with healthcare professionals to identify and eliminate barriers to care.
  • Communicate with reimbursement departments and third-party payers for authorizations.
  • Plan and implement strategies to reduce length of stay and improve patient outcomes.
  • Initiate discharge planning by assessing patient and family needs.
  • Coordinate timely implementation of discharge plans and follow-up care.
  • Perform post-discharge phone calls to verify successful linkage to care.
  • Maintain effective communication with all stakeholders involved in discharge planning.
  • Engage with primary care physicians to support continuity of care.
Requirements
  • Education: Baccalaureate degree in Nursing or related health field from an accredited college or university required.
  • Experience: Two (2) years of experience as a Registered Professional Nurse required.
  • Licenses & Certifications: Valid New York State license and current registration to practice as a Registered Professional Nurse; Basic Life Support (BLS) certification required.
