About This Role

What You'll Do

• Review each patient's chart to ensure documentation supports the plan of care.
• Coordinate and facilitate timely implementation of discharge plans.
• Participate in multidisciplinary rounds to discuss care plans and resource utilization.
• Collaborate with healthcare professionals to identify and eliminate barriers to care.
• Communicate with reimbursement departments to obtain authorizations.
• Plan strategies to reduce length of stay and improve patient outcomes.
• Initiate discharge planning by assessing patient and family needs.
• Maintain effective communication with all stakeholders involved in discharge planning.
• Engage with community organizations to arrange housing and follow-up care.
• Supervise and mentor new care management staff.

Requirements

• Education: Baccalaureate degree in Nursing or related health care field required.
• Experience: 2 years of experience as a Registered Professional Nurse required.
• Licenses & Certifications: Valid New York State license and current registration to practice as a Registered Professional Nurse.
