Residential Transitions, Inc. (RTI) is seeking two Metro Float Mental Health Workers to provide direct support across RTI CRS group homes in the Twin Cities Metro Area.

These full-time DSP roles require travel between multiple residences, offering 40 hours weekly with schedules set at least two weeks in advance; shifts may run mornings to evenings between 6:00 AM and 12:00 AM. You will support individuals with mental illness and related disabilities through person-centered care.

Formación

• At least one year of experience working with individuals with severe mental illness, traumatic brain injuries, or related disabilities.

Responsabilidades

• Provide direct, person-centered support to individuals in RTI's 24-hour residential programs.
• Assist individuals in developing and maintaining independent living skills one-on-one.
• Support individuals in managing mental health symptoms by identifying coping skills.
• Teach and model daily living skills including medication self-management and finances.
• Assist individuals in accessing community resources, transportation, appointments, and recreation.
• Encourage socialization, community integration, and meaningful activities.
• Promote healthy living habits and well-being.
• Follow individualized support plans and documentation requirements.
• Collaborate with program managers and staff to ensure continuity of services.
• Adapt to routines of different programs while maintaining standards of care.
• Provide dependable staffing coverage at assigned locations.
• Perform other duties as assigned.
