Job summary

The Montefiore Hudson Valley Collaborative in Bronx, NY is seeking a qualified addiction counseling professional to provide counseling to individuals enrolled in integrated substance abuse treatment and primary care through the SATP.

Qualifications

• One year’s work experience as an addiction counselor (preferably with methadone clients) and a BA or BS degree required.
• CASAC or license in social work, mental health counseling or a related field is also required.

Responsibilities

• Provide counseling to individuals and families focusing on building motivation, education, planning, crisis intervention, relapse prevention and aftercare.
• Coordinate comprehensive services within the clinic for patients on counseling caseload.
• Enter patient data into HRA/STARS computerized database and maintain accurate progress notes.
• Attend regularly scheduled meetings and participate in supervision to review patient progress.
