Job summary

Harbor Care seeks a MERIT Program Manager to scale the program using evidence-based practices, coordinate referrals, provide intake support, and connect clients with treatment and recovery resources.

The role involves overseeing outreach, staff training, contract/grant activities, and ensuring documentation and reporting are completed accurately. Location is Nashua, NH with frequent travel across NH.

Qualifications

• Minimum of 3 years’ experience in community outreach, referral coordination, intake support, recovery support, or related human services work.
• Current CRSW or LADC certification is required.
• Experience with individuals with substance use disorders, behavioral health needs, housing instability, or related barriers to care.
• Experience with data collection, tracking, reporting, and analytics.
• Ability to collaborate with community partners, referral sources, and multidisciplinary teams.

Responsibilities

• Attend monthly SAMHSA meetings.
• Oversee day-to-day grant activities and reporting.
• Conduct and oversee outreach functions.

Position Overview

The MERIT Program Manager plays an important role in supporting Harbor Care’s mission by scaling the Merit Program using innovative, evidence-based practices, as well as through referral coordination, intake support, recovery-oriented outreach, SUD screening and education as permitted by credentials, and connection to appropriate treatment and recovery resources. This position is responsible for supporting access to substance use disorder, behavioral health, recovery housing, and related support services, while helping create a safe, supportive, and person-centered environment for clients, staff, and community partners.

Location:

Location: based in Nashua, this position will have frequent travel throughout NH. Mileage is reimbursed

Schedule:

Schedule: Monday – Friday, 8:00 am – 4:00 pm or 9:00 am - 5:00 pm

What You’ll Do

Key Responsibilities

• Support referrals, intake coordination, access to services, and recovery-oriented engagement as a member of the SUD Services care team.
• Attend monthly meetings with SAMHSA.
• Oversee all day-to-day grant activities.
• Conduct and oversee outreach functions.
• Develop required policies and procedures.
• Complete required documents and reports.
• Develop and oversee training for staff and community partners.
• Supervise, train, guide, and support new Merit staff in all aspects of the program, including referrals, intake workflows, documentation, billing processes, outreach expectations, and program requirements.
• Build positive and professional relationships with clients, staff, and community partners.
• Maintain accurate and timely documentation, records, or reporting requirements.
• Support Harbor Care’s philosophy of care, mission, and organizational values in day-to-day work.

What We’re Looking For

Required Qualifications

• Minimum of 3 years’ experience providing community outreach, referral coordination, intake support, recovery support, or related human services work required.
• Current Certified Recovery Support Worker (CRSW) or Licensed Alcohol and Drug Counselor (LADC) required.
• Experience working with individuals with substance use disorders, behavioral health needs, housing instability, or related barriers to care.
• Experience with data collection, tracking, reporting, and analytics required.
• Experience working collaboratively with community partners, referral sources, RCOs, and multidisciplinary teams preferred.
• An equivalent combination of education, training, and relevant experience, including military service, internships, volunteer work, and non-traditional career paths, may be considered in meeting position requirements, provided required licensure or certification is met.
• Ability to communicate effectively and work collaboratively with diverse populations
• Reliable transportation for frequent travel throughout New Hampshire
