Job Description:

The Clinical Mental Health Professional utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties. This position provides care appropriate to the established plan of care in the assigned patient population.

Job Specifics

  1. Pay Range Clinical: $34.06-$52.55 Non Exempt
  2. Benefits Eligible: No
  3. FTE: PRN
  4. Shift: Weekdays coverage for sick calls/vacations

Essential Functions

  1. Diagnoses and treats various mental, emotional, and behavioral disorders, modifying care plan(s) to reflect changes in patient status.
  2. Refers patients to other services to improve their well-being. These referrals may include community services, treatment programs, support groups, psychiatrists, or other mental health providers.
  3. Works with the multi-disciplinary team to develop a discharge plan in order to provide further care, services, and referrals in order to ensure continuity of care.
  4. Organizes and prioritizes daily work by assessing new, current, and discharging patient needs in area(s) of responsibility.
  5. Performs behavioral health consultation for patients and caregivers. Conducts psychosocial assessments. Provides supportive, crisis bereavement and other behavioral health interventions as necessary.
  6. Educates, communicates, and coordinates with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely discharge, and to identify and resolve delays and issues.
  7. May provide support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events.

Minimum Qualifications

  1. Master's Degree in Social Work or related field. Education must be obtained from an accredited institution. Degree will be verified.
  2. Current State Licensure as LCSW or LMFT, or CMHC, or Psychologist.
  3. Excellent written and verbal communication skills.
  4. Computer Skills, i.e., email, word processing.

Preferred Qualifications

  1. Experience with patients, families, and treatment teams.

Physical Requirements:

  1. Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
  2. Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  3. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
  4. Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. This typically includes items of varying weights, up to and including heavy items.
  5. Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
  6. May be expected to stand in a stationary position for an extended period of time.
