Qualifications

• Active license in good standing as an Independent or Associate-level Psychologist, Social Worker (LCSW/LMSW), Marriage & Family Therapist (LMFT/AMFT), or Counselor (LMHC/LMHC-A) in NY.
• Experience with adults and at least 1 year of experience working with patients with severe mental illness (SMI).
• Experience in evidence-based interventions, including CBT, DBT, or MI.
• Ability to thrive in a collaborative team environment and build positive relationships with other medical professionals.
• Thorough understanding of confidentiality and documentation standards.
• Technologically savvy with the ability to learn and use new tools.
• Highly organized with the ability to work autonomously.
• Strong listening skills and excellent verbal communication.

Responsibilities

• Provide individual and group therapy to a panel of complex SMI patients.
• Provide continuous, regular support as patients navigate recovery, in person and occasionally virtually.
• Deliver evidence-based care in a holistic, wrap-around care model.
• Collaborate with patient care teams to ensure integrated, cohesive care.
• Address medical needs to mitigate social determinants of health impact.
• Provide consultation to therapists and assist with program development.
