Job Description
GENERAL OVERVIEW
Maintains safety and structure by observing and recording patient information pertinent to the treatment plan and reports information to appropriate staff. Works under nursing supervision.
Essential Responsibilities
- Collaborates with the multi-disciplinary team to provide for an on-going assessment of changes in patient status and gathers pertinent patient care data through rounding and patient interaction using all available resources in the inpatient or outpatient setting. (60%)
- Organizes patient care tasks according to priority and importance of tasks to be completed. (20%)
- Obtains vital signs and height and weight measurements. Provides crisis intervention on the unit to maintain safety. Responds to facility Code events. (10%)
- Reports any changes in physical, mental and emotional conditions to nursing staff and suggests modifications to the master treatment plan. (10%)
Qualifications
Minimum
- Bachelor’s degree or one to two years of relevant experience and/or education as determined by the company in lieu of bachelor's degree.
- Non-violent crisis intervention required within six months of hire
- CPR – American Heart Association
- Act 34 Criminal Background Clearance Certificate
- Act 33 Child Abuse Clearance Certificate
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
Preferred
- 1 - 3 year of mental health or clinical experience
