Job description

Assists R.N. in delivering care, observing patient behaviors, providing daily community meetings and assisting in various procedures. Works closely with the R.N. and reports any changes in patient’s mood or behavior in a timely fashion. When necessary assists the R.N. and unit staff with pre-admission screening calls from referral sources. Also provides information about the program to callers. Patient population ranges from adult through geriatrics.

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Ensures patient safety by observing patient behaviors and completing scheduled rounding.
  • Utilizes verbal de-escalation skills in response to observed patient behaviors, and follows established protocols for safe and effective management of same.
  • Leads or assists with group activities such as community meetings, recreational groups, and other unit activities.
  • Assists RN with patient care by completing vital signs, blood glucose monitoring, EKG, phlebotomy, and other similar activities.
  • Communicates effectively with patients, families, staff, and other customers in written form, telephone conversations, and other verbal interactions.
  • Assists patients with completion of activities of daily living, and encourages independent completion of these activities when possible.
  • Completes required documentation accurately and efficiently, in accordance with hospital policy and department guidelines.
  • Actively participates in unit, hospital, and division meetings and work groups as assigned.

Education:
High School Diploma or Equivalent Required; Bachelor’s degree preferred (12+ college credits in humanities or social services required for Philadelphia work locations).

Experience:
1 year of related experience preferred.
