Job Description

Assists R.N. in delivering care, observing patient behaviors, providing daily community meetings, and assisting in various procedures. Works closely with the R.N. and reports any changes in patient's mood or behavior promptly. When necessary, assists the R.N. and unit staff with pre-admission screening calls from referral sources and provides information about the program to callers. The patient population ranges from adult to geriatrics.

• Interacts with co-workers, visitors, and other staff in accordance with Jefferson's values.
• Ensures patient safety by observing behaviors and completing scheduled rounds.
• Utilizes verbal de-escalation skills and follows protocols for managing patient behaviors safely and effectively.
• Leads or assists with group activities such as community meetings, recreational groups, and other unit activities.
• Assists R.N. with patient care tasks, including vital signs, blood glucose monitoring, EKG, phlebotomy, and similar activities.
• Communicates effectively with patients, families, staff, and other stakeholders through written, verbal, and telephone interactions.
• Supports patients with activities of daily living, encouraging independence when possible.
• Completes accurate and efficient documentation per hospital policies and departmental guidelines.
• Participates actively in unit, hospital, and division meetings and work groups as assigned.

Education: High School Diploma or Equivalent required; Bachelor's degree preferred (12+ college credits in humanities or social services required for Philadelphia work locations).

Experience: 1 year of related experience preferred.
