Job summary

Volunteers of America Minnesota seeks a Mental Health Targeted Case Manager to support adults and youth with serious mental illness in Central MN. You will perform assessments, develop service plans, link to resources, and coordinate care across disciplines in Central MN.

This full-time role requires a BA in behavioral sciences or equivalent and about 2000 hours of supervised clinical experience; travel is required within Central MN.

Qualifications

• Bachelor’s degree in behavioral sciences or related field.
• 2000 hours of supervised mental health service delivery (children/adults).
• Knowledge of local community resources and how to access them.
• Ability to develop person-centered, culturally responsive plans.

Shift Details:FT - 40 hours/week

Schedule:Monday-Friday from 8:30am-4:30pm

Compensation:$49,000-$52,000 annual salary

Location: 2700 1 st Street N, Suite 201, St. Cloud, MN 56303 – includes travel throughout Central MN

About the job:

Maintain a caseload of adult and children clients who have a serious and persistent mental illness (SPMI) or serious mental illness (SMI) and provide services in the four core case management components-assessment, service plan development, referral and linkage to services, and coordinating and monitoring service delivery.

Direct Client Services

• Provide face-to-face and collateral contacts with clients as required by Minnesota TCM standards and individualized service plans, prioritizing in-person services.
• Deliver services that support client access to needed medical, social, educational, vocational, housing, and other mental health services.
• Assist clients in developing and maintaining skills necessary for community living and stability consistent with rehabilitative and recovery-focused principles.
• Coordinate with emergency and crises services in accordance with agency protocol and mandatory reporting laws.
• Travel to service settings, including client homes and community locations, as required for service delivery.

Assessment & Service Planning

• Complete and/or update the Functional Assessment (FA) at intake and at least every 180 days, in accordance to MN DHS standards.
• Develop, implement, and maintain the Individual Community Support Plan (ICSP) that is:
  • Person-centered and culturally responsive
  • Based on the Diagnostic Assessment and Functional Assessment
  • Inclusive of measurable goals and objectives
• Monitor and document client progress towards goals and modify plans to reflect changing needs.
• Coordinate services with the client, family, and multidisciplinary team members in compliance with TCM service requirements.
• Facilitate referrals and linkage to community resources, including but not limited to healthcare providers, schools, housing agencies, and social services.
• Participate in routine clinical supervision, case consultations, team meetings, and interagency planning to support integrated service delivery.

Documentation & Regulatory Compliance

• Complete timely, accurate, and compliant documentation of all services in accordance with MN DHS Rule 79 requirements and agency policy.
• Maintain complete and up-to-date client records, including:
  • Functional Assessments
  • ICSPs
  • Meeting and collateral notes
  • Required authorizations and legal documentation
• Maintain strict adherence to HIPAA and MN data privacy laws (Minnesota Government Data Practices act, where applicable.
• All other duties as assigned.

Required Qualifications:

• Bachelor’s degree in behavioral sciences or related fields or qualify as a Mental Health Practitioner under section 245I.04, subdivision 4.
• Have experience and training in identifying and assessing a wide range of children and/or adult needs.
• Be knowledgeable about local community resources and how to use those resources.
• 2000 hours of supervised experience in the delivery of mental health services to children and/or adults (depending on role), preferred.
