Job summary

Volunteers of America Minnesota is seeking a Mental Health Targeted Case Manager in Central MN. This full-time role combines assessment, service planning and coordination to support adults and children with serious mental illness, with a schedule Mon-Fri 8:30am-4:30pm and travel across Central MN.

You will maintain caseloads, develop ICSPs, connect clients to housing, healthcare and social services, and ensure documentation complies with MN DHS rules and HIPAA.

Qualifications

• Bachelor’s degree in behavioral sciences or related field or qualify as Mental Health Practitioner under MN statute 245I.04.
• Experience and training in identifying and assessing a wide range of children and/or adult needs.
• Knowledge of local community resources and how to use those resources; 2000 hours of supervised mental health services experience preferred.

Responsibilities

• Provide face-to-face and collateral contacts with clients as required by Minnesota TCM standards and individualized service plans.
• Develop, implement, and maintain the Individual Community Support Plan (ICSP) with measurable goals.
• Complete timely, accurate documentation in accordance with MN DHS rules and agency policy.
• Coordinate services with clients, families, and multidisciplinary team members; facilitate referrals to community resources.
• Travel to client homes and community locations as required for service delivery.
